FOID EVALUATIONS
Mental Health Certification for Firearm Possession pursuant to 430 ILCS65/4(a)(2)(iv) and 430 ILCS 65/8(u) are completed by FRWC licensed clinical psychologists who are qualified examiners as defined in 405 ILCS 5/1-122 for solely first responders/veterans.
To conduct an evaluation:

The examiner has to have reviewed ALL collateral information supplied by the applicant prior to assessment.

The examiner has to complete a mental health evaluation including psychological testing and a clinical interview.
Collateral information often needed:

Reasons for revocation including but not limited to police report(s), OOP, civil suits and any legal documentation.

Involuntary or voluntary inpatient hospitalization for mental health.
-
Any related mental health reports and documentation related to danger to self or others.
-
Successful completion of court ordered or voluntary mental health and/or substance abuse treatment.

Letter from IDHS or ISP requesting the FOID evaluation.
REQUIREMTNTS for FOID Certification includes that the licensed clinical psychologist who is the evaluator deems the petitioner is not a serious threat of physical violence against a reasonably identified victim. The evaluator deems the petitioner does not pose a clear and imminent risk for serious injury to themselves or another person. The evaluator deems the petitioner does not demonstrate threatening verbal or physical behavior such as violence, suicidal, homicidal, assaultive threats, actions, or other behaviors.

Common Reasons for losing a FOID:

The person has been committed to a psychiatric hospital due to suicidal or homicidal ideation, intent, or actions.

Clear and present danger to themselves or their community reported by police, mandated reporters, or civilians.

Developmentally disabled or intellectually disabled.

Any domestic violence or OOP.
Questions:
What do I need to share with the FRWC licensed psychologist evaluator?
All of the reasons for the loss of your FOID, including police reports, OOP, hospitalization records, etc. At FRWC, we find that if we do not have all of the records then ISP will often reject your request. So please ensure you send all of your information to our fax at 630-597-2583.
How soon can I get an evaluation?
By law our psychologists cannot schedule an appointment until they have read all of the collateral information. Due to wanting to have success, our psychologists read the background information and will determine if any other documentation is needed and when you can be evaluated. Please send all reports as soon as possible, so we can evaluate and then schedule.
Can a post-doctoral candidate, non-licensed clinician/psychologist complete FOID evaluations? No, they do not meet the criteria under 405 ILCS 5/1-22. Psychologists must be qualified and have at least 3 years of supervised experience after their post-doctorate. It is also unethical for unlicensed psychologists to conduct FOID evaluations. Do your background check on your potential psychologist. You are putting yourself at risk of rejection from ISP when using someone who is not licensed.
